Frontline Assessments, LLC – Terms of Service

These Terms of Service (“Terms”) govern your use of psychological evaluation services provided by Frontline Assessments, LLC (“Frontline Assessments,” “we,” “our,” or “us”). By scheduling an appointment, submitting payment, receiving services, or participating in a promotional offering, you (“Client” or “you”) acknowledge and agree to these Terms. If you do not agree, you may not use our services.
1. Scope of Services
Frontline Assessments provides one-time independent medical opinions (IMOs) for U.S. military veterans for the purposes of VA disability determination. Services are performed by licensed psychologists and are limited to:
- Independent mental health evaluations related to psychiatric or psychological conditions (e.g., PTSD, depression, anxiety, cognitive disorders)
- Structured responses mapped to the VA’s Disability Benefits Questionnaires (DBQs) and narrative reports
- File reviews and second-opinion assessments, when appropriate
- Telehealth-based evaluations using HIPAA-compliant platforms
Note: We do not provide psychotherapy, crisis intervention, or medication management. These are non-treatment evaluations solely intended for documentation purposes. No therapeutic relationship is established.
2. Eligibility
Services are available to:
- Veterans of the United States Armed Forces
- Service members or claimants working with attorneys, VSOs, or authorized representatives
You must be at least 18 years old and capable of providing informed consent. By scheduling services, you confirm the accuracy of your provided information.
3. Appointments, Deposits, and Payment
- Deposit: A non-refundable $100 deposit is required at the time of booking. This will be applied toward the total evaluation fee.
- Remaining Balance: The remaining $615 is due on the day of your appointment and may be automatically charged to the card on file.
- Authorization: By booking, you authorize Frontline Assessments to store and charge your payment method for the remaining balance and any applicable fees (see Section 4).
- We accept credit/debit cards, Stripe, and other secure platforms. You agree to keep your billing information current.
- If we are running a promotional campaign and/or have offered you a discount, we will honor that discounted rate.
4. Cancellation and No-Show Policy
- More than 48 hours’ notice: No additional fees. Deposit is non-refundable but may be applied to a future appointment.
- Within 48 hours: Deposit is forfeited. An additional $357.50 (50% of the full fee) may be charged. This may be credited toward rescheduling.*
- No-show or same-day cancellation: You will be charged the full $715.
- Late arrivals (10+ minutes): May be treated as a missed appointment.
- If we are running a promotional campaign and/or have offered you a discount, we will honor that discounted rate.
5. Telehealth & Technology Use
Evaluations are conducted via secure, HIPAA-compliant video platforms. You are responsible for:
- Ensuring a quiet, private location
- Having a reliable internet connection and functioning audio/video
- Logging in on time with the provided link
6. Confidentiality & Privacy
Your information—including your history, responses, and clinical impressions—will be kept confidential in accordance with HIPAA regulations and our Notice of Privacy Practices (NPP). Your evaluation report will only be released to third parties (e.g., VA, attorney) with your written authorization.
7. Mandatory Reporting & Limits to Confidentiality
In rare cases, your provider may be required to take protective action, including but not limited to:
- Risk of suicide
- Threats of harm to others
- Disclosure of abuse or neglect (child, elder, or dependent adult)
These actions are taken to protect safety and do not affect the confidentiality of the rest of your evaluation.
8. Documentation and Delivery
You will receive a PDF copy of your report within the timeframe discussed during your session. Report delivery timelines may vary based on case complexity.
9. Professional Limitations
Frontline Assessments does not guarantee any specific VA rating or outcome. Clinical opinions are based on interview data, documentation, and professional judgment. We reserve the right to:
- Decline services in cases of potential conflict of interest
- Refer clients to another provider if their needs fall outside our scope
10. Promotional Offerings (When Applicable)
From time to time, Frontline Assessments may offer time-limited promotional evaluations (e.g., pro bono or discounted IMOs). If you are participating in a promotional offering, the following terms apply:
- Non-Treatment Purpose: These evaluations are for documentation purposes only. No therapeutic relationship is established, and no additional services will be provided beyond this one-time assessment.
- Eligibility & Fair Access: Pro Bono or discounted services may be limited by time or volume and are offered equitably, without favoritism. The promotional time frame will be clearly stated on all promotional materials, including the website promotions page, emails, and related communications.
- No Guarantee of Outcome: Participation in a promotion does not guarantee any VA benefit or rating.
- No Obligation for Future Services: No referral, follow-up, or additional service is required.
- No Referral Incentives: Clients will not receive compensation or special access for referring others.
- Truthful Marketing: All promotional communications will reflect services accurately and without exaggeration.
By participating, you confirm that you understand these limitations and consent to the terms of the offer.
11. Modifications to Terms
Frontline Assessments may revise these Terms at any time. Updates will be posted on our website or sent directly to clients with upcoming appointments. Continued use of our services indicates acceptance of any updates.
